To enroll your child in our center, you may call 513-427-4271 for a pre-admission interview or you mail email the administrator to schedule a pre-admission interview at growinginfaith@monumentsbaptist.org .

Please check before filling out these pages that there are openings in your child's age group! You may speed up the enrollment process by printing and filling out these forms. When you come for a tour and pre-enrollment interview then you will recieve our handbook.

**We are ONLY allowed to put infants on their backs, if you would like us to put your infant in another position then you must complete the Sleep Wavier form. http://www.odjfs.state.oh.us/forms/file.asp?id=183&type=application/pdf

(Everyone) Child Medical Statement form. This must be completed and turned in within 1 month of enrollment. http://www.odjfs.state.oh.us/forms/file.asp?id=240&type=application/pdf

(Everyone) Parent Information. This is just a form we are required to give you by Ohio Job and Family Services.

**Do we need to give your child any medications (dr. perscribed or over the counter) or put any topical ointments (sun lotion, diaper rash ointment, etc) on your child? You MUST complete a Request to Administer Medication Form for EACH medication/ointment. Your child's name must be on the medication/ointment and placed in a zip loc bag. If your child's weight/age is not listed on the bottle (ie most infant age/weights are not listed) then you must have the doctor complete section B on the form. Section A may be completed by you for all other medications.
